In Kazordoon, a short walk south of the temple (44 paces), 2 levels below ground (find a cave or stair entrance and head down).
The area is guarded - expect Slimes, Minotaur Mages, Minotaur Archers, Dwarfs.
Recommended: level 20+.
Open the quest chest to claim the reward; each character can loot it once.
