Travel far south-west from Thais (264 paces from the temple), 3 levels below ground (find a cave or stair entrance and head down).
The area is guarded - expect Dragon Lords, Dragons, Demon Skeletons, Beholders.
Recommended: level 80+.
Open the quest chest to claim the reward; each character can loot it once.
