Head south from Kazordoon's temple, about 206 paces, 1 level below ground (find a cave or stair entrance and head down).
The area is guarded - expect Slimes, Fire Devils, Minotaur Archers, Scorpions.
Recommended: level 20+.
Open the quest chest to claim the reward; each character can loot it once.
