In Ankrahmun, a short walk south of the temple (81 paces), 8 levels below ground (find a cave or stair entrance and head down).
The area is guarded - expect Mahrdis, Dragon Lords, Dragons, Necromancers.
Recommended: level 80+.
Open the quest chest to claim the reward; each character can loot it once.
