In Kazordoon, a short walk north of the temple (53 paces), 3 levels below ground (find a cave or stair entrance and head down).
The area is guarded - expect Dwarfs, Rotworms, Rats.
Recommended: level 8+.
Open the quest chest to claim the reward; each character can loot it once.
