Travel far north-west from Carlin (330 paces from the temple), 2 levels below ground (find a cave or stair entrance and head down).
The area is guarded - expect Beholders, Minotaur Guards, Minotaur Mages, Minotaur Archers.
Recommended: level 20+.
Open the quest chest to claim the reward; each character can loot it once.
